Budgeting for long-term GLP-1 treatment: a simple plan

If you’re trying to budget for long-term GLP-1 treatment, you’re probably feeling the squeeze right now—prices can jump, coverage rules can shift, and even availability can be unpredictable. Whether you’re using Ozempic, Wegovy, Mounjaro, Zepbound, a GLP-1 pill, or a compounded option, it’s normal to feel uncertain about what next month will cost. This article is for education only, not medical advice.

1

Step 1: Map your “all-in” monthly cost (not just the drug)

Start with a simple list: medication cost, clinic or telehealth visits, lab work, needles/alcohol swabs if you use injections, and shipping or pharmacy delivery fees. Add the cost of missed work time or transportation if that applies to you. This gives you a realistic monthly baseline. Keep the list in one note so you can update it when anything changes.

2

Step 2: Separate coverage paths: insurance, cash-pay, and bridge options

Write down which path you are using right now: commercial insurance, Medicare Part D, self-pay, or a mix. If you have Medicare Part D and meet eligibility, the Medicare GLP-1 Bridge (running July 1, 2026 through Dec 31, 2027) may offer a $50/month copay for Wegovy (all forms), Foundayo, or Zepbound KwikPen, with prior authorization requirements. If you use commercial insurance, ask your plan which GLP-1 products are on formulary and what the prior authorization rules are. Do not assume last month’s coverage will be the same next month—set a reminder to re-check when the plan year or your dose changes.

3

Step 3: Compare form factors that change the budget math

Different forms can affect convenience and cost structure, even when the active medicine is similar. As of Oct 30, 2026, Wegovy pill (oral semaglutide 25 mg) is available and must be taken first thing in the morning with a small sip of water, at least 30 minutes before any food, drinks, or other oral medicines—this timing can affect routine and adherence costs (like missed doses). Foundayo (orforglipron) is a once-daily GLP-1 pill that can be taken any time of day with no fasting or water restrictions. Zepbound KwikPen is a multi-dose pen that holds a month of doses, which can simplify refill timing and reduce last-minute pharmacy runs.

Step 4: Use manufacturer channels to confirm “at the time of writing” prices

If you are self-paying or your coverage is unstable, build your budget from verified sources you can re-check quickly. At the time of writing, TrumpRx.gov lists the lowest manufacturer self-pay prices and routes you to the maker’s channel (for example, LillyDirect or NovoCare); it is not insurance and listings can change often. Foundayo is available via LillyDirect, telehealth, and retail pharmacies, and at launch had self-pay starting at $149/month for the lowest dose, with a commercial savings card as low as $25/month for eligible patients—confirm current eligibility and pricing directly with the manufacturer. Always screenshot or save the terms you see so you can compare later if the program changes.

5

Step 5: Build a “prior authorization and refill buffer”

Delays cost money when they lead to urgent visits, rushed shipping, or gaps you did not plan for. Create a calendar system: mark refill request dates 10–14 days early, your next follow-up visit, and any prior authorization renewal deadlines your plan requires. Keep a small buffer fund for one-time costs like an extra visit, updated labs, or a temporary cash fill if your plan is processing paperwork. If you are considering compounded versions, treat them as a separate budget line item and ask your pharmacist and prescriber how they handle continuity, documentation, and refills.

6

Step 6: Plan for life changes: pregnancy, contraception, and switching costs

Long-term planning includes the possibility that you may need to pause or change treatment for personal or medical reasons. GLP-1 medicines are generally not recommended during pregnancy, so if pregnancy is possible, build time and appointment costs into your plan and discuss options with your prescriber or OB-GYN. If you use tirzepatide (Mounjaro or Zepbound), it can reduce oral contraceptive absorption around starts and dose increases—questions about contraception timing and alternatives should go to your clinician. Also plan for “switching friction,” such as new prior authorizations, new device training, or a different pharmacy, because these transitions can add unexpected costs.

Think of GLP-1 treatment like an ongoing monthly bill—with a few surprise costs now and then—so plan for it to change a bit over time, not stay perfectly steady. Bring your prescribing provider a short list of questions like: “What will my typical monthly cost be with my insurance?”, “What could make it go up?”, and “When should we re-check coverage, prior authorizations, or savings programs?” Today, take one small step: write down your best estimate of your all-in monthly cost.
